A task force comprised of Connie Everett, Susan Kindle, and Dr. Divya Shroff developed a new electronic patient discharge instruction template that makes it easier for patients to follow their doctors’ post hospitalization instructions. The team won first place in the VA Under Secretary for Health’s National Customer Service Award program. The new instructions were developed to improve patient understanding of their post discharge health care needs. By analyzing patient feedback from the National Survey of Healthcare Experience of Patients (SHEP) the team was able to target issues of concern. These included using larger type, using basic language and providing simpler instructions related to medication use after discharge. This new template will be adopted throughout the nation and is an example of system improvements that can be made when you take into consideration patient feedback. The new template will be incorporated into the award winning VA electronic patient records system.
