The Washington DC VA Medical Center's Patient Safety Office has been recognized with the Gold Cornerstone Award for the third year in a row for its patient safety initiatives by the Department of Veterans Affairs National Center for Patient Safety (NCPS). NCPS initiated the Cornerstone Recognition Program in 2008 to recognize patient safety leaders and to enhance the Root Cause Analysis process. Gold, Silver and Bronze level awards are presented to the VA medical centers which meet the criteria for quality, timeliness and quantity of its Individual Root Cause Analyses (RCA) and Aggregate Reviews. DCVAMC was also recognized in 2009 with the Silver Cornerstone Awards.
