If you’ve visited the Washington DC Veterans Affairs Medical Center recently, you’ve noticed the latest construction project near the front entrance. The new Transition and Care Management Center opened April 10 to offer a multitude of services to transitioning Veterans in a “one-stop” resource shop.

According to TCM Program Manager Elsie Moore, the new TCM Center is a big improvement for injured and/or ill combat (OEF/OIF/OND) and Post 9/11-era Veterans. The new space is much larger and offers more patient privacy and efficiency for staff and Veterans alike. “Veterans are able to find us and many of the other services they need in one location,” Moore said.

The TCM office was formerly known as the Operation Enduring Freedom, Operation Iraqi Freedom, Operation New Dawn Program. The program name changed last year but its mission, to provide hands-on care management services to address physical, emotional, and social concerns associated with transitioning to civilian life, remains unchanged.

Co-located within the new TCM Center will be: (please note some community and partner services are only available on a rotating basis)

Release of Information:  The ROI office will be providing individual medical records on a disc upon request. Drop-off requests are welcome.

MyHealthevet (MHV) Coordinator:  The award-winning online program provides Veterans with 24/7 access to trusted health and benefits resources including pharmacy refills, appointments and secure messaging with providers.

Eligibility:  Helps establish Veteran’s eligibility for VA services and benefits.

The Veterans Pro Bono Legal Clinic: Attorneys will be available to low-income Veterans to discuss issues and legal referrals in the following practice areas: family, Veteran, landlord/tenant, benefits, consumer, military discharge upgrade matters and employee law. The legal clinic will be open every Friday from 10:00 a.m. – 4:00 p.m. Appointments are available and walk-in times are from 12:00-12:30 & 2:00 - 2:30 p.m. 

Financial Counseling Clinic: Services available the second Monday of each month from 10:00 a.m. – 2:00 p.m.  (By appointment only)

Veterans Benefits Administration/Vocational Rehabilitation & Education: Provides information sessions each month about the programs:

Starting May 24:

Every second Wednesday of the month from: 9:00 a.m. - 10:30 a.m.

And every fourth Wednesday of the month from: 1:00 - 3:00 p.m.

 (Please note: There are only 10 seats available per session. Please, call the TCM Center to sign up for a session.)

Caregiver Program for Post 9/11 Veterans: By appointment only.
